Application Procedures

Call our office at (213) 273-7174 and request an application, or download one from the link below. Once the application is complete, mail it to: DC Teen Discipleship, 2301 Bellevue Ave, Los Angeles, CA 90026.

Once we receive a copy, you will be placed on a waiting list. Our reception of your application does not guarantee that your child will be placed. Contact us to determine how long you might have to wait before a space opens.

When a space becomes available, your family will be contacted for an interview. The results of that interview will help us to determine whether placement would be suitable. We place children based one several factors, including, but not limited to, age of children, seriousness of behavior problems, current social make-up of the home, available scholarships, quantity and quality of available staff.

If your child is accepted for admission, you will receive an acceptance packet from us that includes the contract, a physical form, health insurance intructions, and information about what to bring.
